2/28/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted Madison upon arrival to the home setting. Upon arrival to the home setting, Madison appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHP-C informed Madison of the focus on todayâs session which was going to be continue to focus on her ISP goals which one of the goals is to assist her with being able to manage her anger and respect those who are in authority. 2. Behavior: Madison reported that she will try to apply techniques.. Madison went on to report that she was chill and after todays session. Madison reported during this part of the session she is angry when she cannot have her way or when she is told "No by her mother". 1. Intervention: Upon arrival to Madisonâs home for todayâs session, QMHP-C assessed and monitored Madisonâs mood and behavior. QMHP-C probed in order to process and explore Madisonâs aggression towards mother and others. QMHP-C reviewed and discussed anger management skills with Madison to improve the way she communicates to mother, family members and others. 2. Intervention: QMHP-C allowed Madison to express her thoughts about her behaviors along with brainstorming some of the things she would like to change that are triggering her reported behaviors. QMHP-C provided Madison with feedback. QMHP-C asked questions along Madison to ask questions, QMHP-C encouraged reinforced the importance of exhibiting positive and respectful behaviors within the home, school, and community setting. 1. Response: Madison reported that she became upset when her aunt yelled at her about not cleaning up the Kitchen. Madison stated that she was upset about not be able to go bowling with her friends. Madison was observed being disrespectful to Ms. Spencer during todayâs session. Madison continue to talk back towards Ms. Spencer while talking to QMHP-C. Madison reported that she would stop after being directed to stop by QMHP-C. Madison reported that she will not cause any more problems after talking to QMHP-C. 2. Response: Madison reported that she will try to apply techniques. Madison went was able to understand how her having positive behaviors can have a major impact on her day when she allows herself to be in control of her own emotions.
